By accessing neuform-fitness.com (the “Site”), creating an account, using any NeuForm tools or content, or purchasing any NeuForm product or service (collectively, “Products”), you agree to these Terms and our Privacy Policy (together, the “Agreement”). If you do not agree, do not use the Site or purchase Products.
These Terms apply to all current and future NeuForm offerings, whether delivered digitally, physically, or otherwise, unless different terms are expressly provided at checkout or within a specific product offering.
NeuForm Products are intended for adults (18+). If you are under 18 (or the age of majority where you live), you may only use the Site and Products with the involvement and consent of a parent or legal guardian.
If you are a parent or legal guardian and you allow a minor to use the Site or Products, you are the purchaser and responsible party, you agree to this Agreement on the minor’s behalf, and you accept full responsibility for supervision, use, and outcomes.
The Site is not directed to children under 13, and children under 13 may not use the Site or Products.
Definitions. “Products” may include, without limitation: downloadable training plans and guides; subscriptions and memberships; online tools, software, dashboards, and calculators (including nutrition/hydration tools); custom plan services; educational content; and any future offerings (including books and other digital or physical resources).
Educational use only. All NeuForm content, tools, and Products are provided for educational and informational purposes only. NeuForm is not a medical provider, registered dietitian practice, psychotherapy provider, or mental health service, and does not provide medical advice, diagnosis, treatment, or therapy.
Tools & calculators. Any calculator, recommendation, template, automated output, generated plan, or other result is an estimate based on your inputs and assumptions. You are responsible for your decisions and for verifying outputs with qualified professionals as needed.
DLTER / self-development content. DLTER and related content are conceptual educational frameworks intended for self-reflection and personal development. They are not mental health services, clinical evaluation, diagnosis, or treatment.
No guarantees. NeuForm does not guarantee any specific fitness, strength, aesthetic, body composition, nutrition, or performance results. Individual outcomes vary based on training history, genetics, recovery, nutrition, adherence, and many other factors.
Testimonials. Testimonials and examples reflect individual experiences and are not promises of typical or guaranteed outcomes.
Training and physical activity involve inherent risks, including the risk of serious injury or death. You are solely responsible for your health and safety while using the Site or Products. Consult a qualified healthcare professional before beginning any exercise or nutrition program, especially if you have a medical condition, injury, are pregnant, or are taking medications that affect exercise tolerance, appetite, blood pressure, or metabolism.
You voluntarily assume all risks arising from your participation, including risks associated with exercises, equipment, environment, and any reliance on content or tool outputs.
Certain Products or features may require an account or gated access (e.g., Memberstack). You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account.
You may not share accounts, login credentials, or gated access with others. Unauthorized sharing may result in access termination without refund.
NeuForm digital Products are digital-only unless explicitly stated otherwise. When you purchase a digital Product (e.g., training plan PDFs, guides, templates, tools), you receive a personal, non-transferable, non-exclusive, revocable license to download and use it for your personal, single-user use.

Some Products are custom services (“Custom Plans”) and require you to complete an intake questionnaire. The Custom Plan service begins at purchase because access and processing are initiated immediately.
Time-limited intake. Unless otherwise stated on the Product page or checkout, you have a limited time window to submit your intake after purchase. If you do not submit your intake within the stated window, the order may be considered used/fulfilled and may require a new purchase to restart.
Automated + manual review workflow. NeuForm may use automated scripts to generate an initial plan draft based on your intake. Plans are then manually reviewed before final export and delivery where applicable.
Delivery timeline. Estimated delivery timelines (e.g., “within ~12 hours”) are targets, not guarantees, and may vary due to workload, technical issues, or circumstances outside our control. If delays occur, we will make reasonable efforts to deliver as soon as practical.
Accuracy of information. You are responsible for providing complete and accurate intake information. NeuForm is not responsible for outcomes arising from inaccurate, incomplete, or misleading information.
Some Products may be offered as subscriptions (“Subscriptions”). Subscription terms (pricing, billing interval, included features) will be displayed at purchase.
Prices may change without notice. Payments are processed by third-party providers (e.g., Stripe, Memberstack). NeuForm Fitness LLC does not store full payment card numbers.
Digital Products may be delivered by on-screen download, gated access, and/or email link after purchase. You are responsible for providing a valid email address and ensuring you can receive messages from us.
If we offer physical goods (for example, a printed book or merchandise), shipping fees, delivery estimates, and any product-specific terms will be shown at checkout or on the product page.
If you have access issues, contact us within 7 days of purchase and we will make reasonable efforts to provide a working copy or restore access. Any resends, extensions, or accommodations beyond this window are provided at NeuForm’s sole discretion and are not guaranteed.
Certain Site features may rely on third-party platforms such as Memberstack, Webflow, Stripe, Google Workspace, and Google Forms/Sheets. Your use of those features may also be subject to those providers’ terms and policies.
General rule: Because many Products are digital and/or involve immediate access or services that begin at purchase, sales are final except as required by law or as expressly stated here (or as otherwise stated at checkout for a specific physical product).
Once a digital Product is delivered or access is granted, refunds are not provided for reasons such as change of mind, lack of use, user error, or subjective dissatisfaction.
Custom Plans are non-refundable once purchased because the service begins at purchase and includes time-sensitive access and processing. Failure to submit the intake within the stated window may require repurchase to restart. Any exceptions are at NeuForm’s sole discretion.
Subscription charges are generally non-refundable for the current billing period. You can cancel to prevent future renewals. If you cancel, you retain access until the end of the current period unless stated otherwise at checkout.
If we offer physical goods, any return, replacement, or shipping-issue terms will be disclosed at checkout or on the product page and will control for that item.
If there is a technical problem (e.g., corrupted file, failed access, duplicate charge), contact us within 7 days and we will correct the issue (for example, by restoring access, resending a file, or refunding a duplicate charge where appropriate).
Initiating a chargeback without first contacting us to resolve the issue may result in loss of access and may be treated as a breach of this Agreement. We reserve the right to provide payment processors with relevant order and access records to contest chargebacks.
All content on the Site—including plans, text, graphics, logos, videos, tools, templates, and code—is owned by NeuForm Fitness LLC or its licensors and is protected by copyright and other laws. Except for the personal license granted above, no rights are transferred or granted.

If you submit a testimonial or review, you grant NeuForm Fitness LLC a non-exclusive, worldwide right to display, reproduce, and use it for marketing and promotional purposes. We may edit for clarity and length without changing the meaning.
We will display only your first name and initial unless you request anonymity. Testimonials reflect individual experiences and are not guarantees of typical outcomes.
The Site may reference or link to third-party services. NeuForm is not responsible for third-party content, policies, availability, or practices. Your use of third-party services is governed by their terms and policies.
The Site and Products are provided “as is” and “as available.” To the fullest extent permitted by law, NeuForm Fitness LLC disclaims all warranties, express or implied, including warranties of merchantability, fitness for a particular purpose, and non-infringement.
To the maximum extent permitted by law, NeuForm Fitness LLC (and its owners, affiliates, and service providers) will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages, or for lost profits, data, or goodwill.
Our total liability for any claim arising out of or relating to the Site or Products is limited to the amount you paid for the Product or $50, whichever is greater.
You agree to defend, indemnify, and hold NeuForm Fitness LLC harmless from any claims, damages, liabilities, losses, and expenses (including reasonable attorneys’ fees) arising from your use of the Site or Products, your breach of this Agreement, or your violation of law or third-party rights.

We may modify or discontinue Products, features, or pricing at any time without notice. We are not liable for any modification, price change, or discontinuation.
NeuForm Fitness LLC is not responsible for delays or failures caused by events outside reasonable control, including outages, platform/provider interruptions, internet failures, natural disasters, labor issues, or governmental actions. If such events occur, we will make reasonable efforts to resume delivery and access.
These Terms are governed by the laws of the State of Illinois, without regard to conflict-of-law rules. You agree to exclusive jurisdiction and venue in the state courts of Will County, Illinois, and the federal courts of the Northern District of Illinois.
We may update these Terms periodically. The “Effective date” at the top of this page will be revised accordingly. For material changes, we may post a notice on the Site. Your continued use of the Site after any changes constitutes acceptance of the updated Terms.

Our providers may process data in the United States and other countries. Where required, transfers rely on recognized safeguards (e.g., Standard Contractual Clauses) offered by those providers.
Some workflows may use automated scripts to process your submitted inputs and generate draft outputs (such as preliminary training plan structures) before any manual review where applicable. These outputs are educational estimates based on inputs and are not medical or therapeutic decisions.
If you believe your submitted information was incorrect or you have questions about an output, you may contact us to correct errors or request guidance on next steps.
Depending on your location, you may have rights to access, correct, delete, restrict processing, object, port your data, and withdraw consent. To exercise your rights, email chriswelch.neuform@gmail.com. We will respond as required by law.
You may request to know, delete, or correct personal information, and opt out of “sale” or “sharing” where applicable. NeuForm does not sell personal information. If our use of certain analytics/advertising tools constitutes “sharing” under CPRA, you may opt out through available controls or by contacting us with “CPRA Request” in the subject line.

If you are in the EEA, UK, or Switzerland, you have the rights listed above and may lodge a complaint with your local supervisory authority. Our lawful bases are listed in Section 3.
